Has there ever been a time where since the 2013 rebrand, they have played out the 2009 bed/theme/closing music/titles by mistake?
The lunchtime bulletin just now with Mary Nightingale just ended with the regional closer theme. Think this has happened before for the short bulletins?

That would be impossible. The old music was removed from SpotOn in 2013 and the actual audio files have since been archived. The reason why it's easier to play out the current regional theme by mistake is because it is also loaded onto SpotOn because of the London regional news that comes from the same control room.

Yes. During the 1800 London bulletin, post weather is pre-recorded so they can prepare for the Evening News. You'll also notice that Saturday evening's London bulletin utilises the same presenter and set, so when they go to a break, they just change the background to change to the network bulletin. The same happens during the ITV Lunchtime News, so they can switch to London after.

The SpotOn system has different 'pages' of buttons for music cues for each programme, so if the incorrect page was loaded, the wrong closing music might be played out in error.

Are ITV News providing a Scotland only late bulletin this year before STV and ITV goes it's separate way or is the last one for us for 2017 at 6.35pm?

In contrast to BBC1, where Scotland got a bulletin at 21:30, with the rest of the network getting it at 22:00.
I'm assuming there will be no regional bulletins on ITV in England on New Years Day? The presenter on STV news said they would return on the 2nd January.
