Spare a thought for those of us in the North East; we get Tyne Tees on 103 (ITV1), YTV on 131 (ITV1+1) and Granada on 178 (ITV1HD).

It has indeed always shown ITV1+1 Wales. I think the original label was because they thought of it as a 'West-UK Macro Region', which of course upset those in the former ITV West & ITV Westcountry regions, so the label was dropped at the same time as all the others for ITV1+1.

It was still Scottish and Grampian in 2006, then switched to STV in 2007. But it's just 'STV' on all of them. You don't really know which one you're watching until the 6pm news comes on.

How many people actually get the "wrong" region on BBC1 and ITV1... and how much is this about anoraks like us wondering exactly what they are getting in the 800s?
In the English regions, regional boundaries have always ultimately been about transmitter footprints and not editorial factors. If at the margins there are a few places which have now, in effect, moved from one region to the other then the regional news teams and advertising micro regions can take account of this. Say if there's a place in North Yorkshire where most people used to watch Bilsdale but where, post switchover, boxes put YTV (sic) on 3. How many people at the margins of coverage really mind moving to a different news region? The problem would be if there local area wasn't covered at all. (Editorially both the Leeds and Newcastle programmes were always aware that there were areas where people could have watched been watching either.)
I know there are a few places which, editorially, should get STV East but where the STV West signal dominates - and vice versa. But the problem is, really, where people in one nation get programmes from across the border. Not only are the news areas formally defined, but the schedule varies enough to cause confusion.
It's potentially confusing on Sky to see "brands" which are effectively dead on the EPG on 103 far less sub-regions which now only carry local adverts. Eg ITV1 Meridian SE, ITV1 TT S.
